The US and Russian forces had declines and been targeted at sea areas. It would still have been devastating. At best, we could see a "Fail-Safe" incident involving a small exchange of nuclear force. Worst-case scenario: Nuclear WWIII, with the PRC and Israel as the preeminent nuclear powers. Russia would be worse off than the NATO powers, but NATO nations would lose cities of people and the world economy would sink.
